Service Survey Questionnaire
(World War II, Korean War, Vietnam & The Cold War)
The U.S. Army Military History Institute, a component
of the Army Heritage and Education Center, collects information
from veterans about their experiences in the form of a written
survey. The survey program has been underway for many years
and includes surveys from veterans beginning with the Spanish-American
War.
The survey seeks to document, by means of this questionnaire,
the recollections and opinions of veterans of the United
States Army.
Responses to the survey are important to researchers and
historians.
Surveys may be obtained by calling the Military History
Institute staff at 717-245-3971 or surveys may be printed
from the Military History Institute website at:

Veterans Survey Facts
- The archive collection of the Military
History Institute contains the personal materials and
papers of soldiers of all ranks.
- Researchers, historians, and authors
use the archive collection that includes the veterans
surveys.
- Veterans may include other
items of historical interest with their completed survey.
Donation of Materials
The AHEC, MHI, and the Army Heritage Museum accept donations
of items of significant historical interest, which include
personal papers, letters, scrapbooks, diaries, memoirs,
photographs, correspondence with family and friends, camp
and unit newspapers, regulations and manuals, unit histories,
and certain other books, professional military journals,
maps, soldier art, unit insignia, uniforms and other items
of militaria.

The Army Heritage Center Foundation does not accept donations
of archival materials or artifacts.
